Tournament Fee Request Form

This form is for Team Managers to complete to request funds from MLMHA to cover their tournament entry fees prior to having their rosters finalized and team fees set.  Please note that any funds that are paid out on behalf of a team by MLMHA must be repaid in full to MLMHA by December 1.
