Informal incident reports are intended to deal with minor infractions of the applicable Code of Conduct or a breach of policies. Examples would include disrespectful conduct that does not reach the level of bullying, harassment, or abuse, but is still not in line with MLMHA's goals of respect, tolerance, fair play, equality, and diversity.
In the case of an informal incident report, the complainant will not be advised of the outcome. The person(s) complained of will be contacted, outlining that a complaint has been made, the matter complained about, and recommended steps to resolve the complaint. Note that in the case of an informal complaint, the complainant will not be identified.
Please be sure to review the MLMHA Codes of Conduct and the MLMHA Policies and Procedures Manual before submitting your report.
