CRC Letter Request

This form is for anybody who is in a volunteer position with MLMHA the requires a CRC.  Please submit the following form and upon approval, you will receive an 'Intent to Volunteer' letter from MLMHA which can be brought to your local RCMP detachment along with your CRC application.
